Autism Waiver Service Coordination
As Required by DHMH/MSDE - Includes 2 Components:
Assessment
- Facilitates eligibility process including financial, technical, and medical
- Coordinates and completes initial assessment and annual reassessment
Ongoing
- Monthly contact with families to review satisfaction of services
- Quarterly visits with participant (one visit per year must be in-home)
- Updates Plan of Care as needed
- Assist family in obtaining Environmental Accessibility
- Adaptations and Durable Medical Equipment
- Central contact person with MSDE, DHMH and FCPS
As Provided by The Arc of Frederick County:
- Provide information and referral to other resources
- Monthly informational letter to families
- Assists family to complete MA redetermination application if needed
- Help family to complete DDA Application
- IEP Advocacy
- Problem solve with families
